Blood, and the system that moves it

Blood cannot be manufactured, stored indefinitely or ordered at short notice from abroad. A transfusion service is therefore a continuously running logistics operation, and demand in this period rose sharply.

What the system requires

A donor base large enough to supply regularly, since red cells keep for weeks and platelets for days. Testing capacity to screen every unit. A cold chain that holds from collection to transfusion, including through power cuts. And distribution that gets the right blood group to the right hospital before it expires.

Every one of those was under pressure simultaneously.

What was reformed

Consolidation of a fragmented system into a coordinated national service, which is a reform that had been discussed for years and was implemented under pressure. Centralised information on stocks, so a hospital short of a group can be supplied from a centre that has it rather than issuing an emergency appeal.

Modern testing and processing equipment, largely supplied by partners.

The donors

Donation rates rose substantially and, importantly, the composition shifted towards regular voluntary donors rather than emergency responses to appeals. A service that depends on appeals is unstable; one with a regular donor base is a system.

The observation

This is a reform that improves ordinary healthcare permanently. Long after this period, a person needing surgery in Ukraine will benefit from a blood service reorganised during a war, and will never know it.
