A hospital needs power before it needs anything else

The winter of 2022 to 2023 was defined by attacks on the grid, and the institution least able to tolerate a power cut is a hospital.

What the priority actually was

An operating theatre, an intensive care unit, a neonatal ward, a dialysis centre and a laboratory freezer all fail in different ways when power stops, and some of them fail permanently. A vaccine store that thaws is a total loss. A ventilator without power is an emergency measured in minutes.

So the most valuable medical assistance in this period was electrical: generators sized properly for a hospital rather than a house, fuel supply arrangements, automatic transfer switches, and batteries for the equipment that cannot tolerate even the seconds a generator takes to start.

The supply chain half

Medicines and consumables continued to arrive in volume, coordinated so that hospitals received what they actually lacked rather than what was easiest to send — a distinction that matters more than it sounds, because uncoordinated donation produces warehouses full of the wrong thing.

Field medicine

Training in tactical and trauma medicine expanded considerably, delivered both by visiting instructors and by Ukrainian clinicians trained abroad who returned to teach. Stabilisation points closer to where injuries occur shortened the time to surgical care, which is the single variable that determines survival in trauma.

What I would record about this quarter

The system did not collapse in the winter it was most likely to. That outcome was bought with equipment lists, logistics and unglamorous planning rather than with anything that photographs well.
